Patient Billing
Our goal is to get you back to orthopedic wellness and a mutual financial understanding is a part of our relationship. For your convenience we accept Visa, MasterCard, AMEX and Discover.
We participate in most health plans. You will need to call your plan and verify if we are In Network. If we are Out of Network you can still see us, but will likely have a higher out of pocket cost. If you have an HMO plan, you will need authorization to see us, usually granted by your primary care provider. If you do not have authorization and see us, you will be considered a self pay patient. We request payment in full at time of service for self pay patients. We do not take auto insurance plans if you were in a motor vehicle accident. You would be considered a self pay patient and you would submit the required documents to your auto policy for reimbursement. If you have a workers compensation claim, we will need written authorization from your adjuster. If you were injured while playing sports and have school or sports team medical coverage, please bring that information with you along with your regular insurance plan(s).
Charges not covered by your insurance company that may include co-payment, co-insurance, deductible amounts and those services where authorization was not obtained are your responsibility.
We will send you a monthly statement. Any remaining balance after your insurance has paid is your responsibility. Your prompt remittance is appreciated.
